- Gender identity and Autism - helping clients with transitioningIn Conditions + Specialisms·16 November 2020I presently work with a client who has ASD, is transgender (M to F) and experiences significant anxiety disorder. I work extensively on social skills as well as IADLs and ADLs, however skill development remains a challenge due to anxiety and an impairment in abstract thought and insight. I use a fair amount of cognitive adaptation strategies (signs, lists, etc) for managing anxiety, but I am finding it quite difficult to find resources/visuals that are appropriate for teaching an adult with ASD about transitioning and self-care skills. If anyone has any suggestions or links to resources, they would be very appreciated!2364
- Service user involvement.In Service Development·2 August 2017Where I work service users are involved in various areas of the hospital's Clinical Governance processes such as recruitment, service user forums, training of staff. We have ward representatives who have the opportunity to attend other meetings and input into the running of the hospital however I struggle to get commitment in their attendance. - Out and About Group - Mental Health RehabIn Approaches + Frameworks·5 November 2022Hi everyone, I'm a new band 5 on rotation in an adult mental health rehab unit. I've been asked to run an Out and About Group utilising the Recovery through Activity program. The group was previously run by an ATP. I'm just curious if anyone else has working in MH rehab and what the focus of groups should be? Is it participation in activity or to work towards specific goals of the individuals to promote longer term changes in functioning? I don't want to be seen as just an activity coordinator? My idea for the planning session will be providing some education and encouraging discussion around the benefits of accessing community and social inclusion. Any advice would be much appreciated.23115

- Covid19 mental health and leisure/quality of life tx and tx difficultiesIn Approaches + Frameworks·10 February 2021HELP. Am I missing something? I work in a SNF with several locked dementia units and regular SNF unit. Since covid, I feel, I've been having to act as a rec therapist making activity folders for people who no longer know their name and also provide mental health tx for both advanced dementia and the regular SNF unit. Pre covid our awesome activity dept. was on these units daily with many group activities with several staff. Some of these things OT could do but there wasn't a need as it was already being done. Now somehow I'm supposed to re create all that, even though the activity dept has had to severely change and limit its operations. I can't fix this, I can't fix dementia and so I struggle when someone needs constant 1 to 1 assist for participation in a leisure activity (that they can't even identify). I'm at a loss. I often feel like an asshole taking these communication forms, with activity suggestions and directions for set-up, to the CNAs and nurses. Patients need to be fed, provided with hygiene and have PROM HEPs completed. With all these things, 1 of 2 CNAs for 25 residents, is not able to complete a leisure activity, 1 to 1, for 30 minutes everyday. I feel like no one is recognizing limited staffing and environmental limitations. HAVE I GONE MAD OR MISSING SOMETHING MAJOR? And when I express these concerns I'm simply told I'm not giving these patient's a "chance." The list of crazy reasons for therapy and expectations for OT to fix them goes on and on. Improving participation and IND in self feeding is very difficult when pt's with advanced dementia can't leave their rooms and be in feeding dining rooms. I also feel like my company wants me to be a full on counselor, but this is another profession entirely and so I'm struggling with where the line for OT mental health and a counselor or psychologist is? Especially with dementia pts. I address through ther act, some staff training, making sure meds for anxiety /depression are part of tx and educate on coping strategies as cognitively appropriate. Lastly, it is always expected that natural aging declines are instant therapy referral. There are times I think this is cruel and highly in-appropriate and nursing staff agrees (at times disgusted), but again my company does not. To them, end of life is not a reason to not provide therapy. I cover comfort measures. And this has become more and more of an issue since covid began. I would love any suggestions, thoughts or constructive feedback. I have no OTRs to discuss these issues with as all my superiors are DPTs23118

I am an MSc pre-reg OT student writing a dissertation about home-based stroke rehabilitation services. The RCP (2016) guidelines for stroke suggest occupational therapy input improves PADL/EADL outcomes for service users, but there seems to be evidence lacking with regard to the content of therapy and understanding which interventions are most effective. I am interested to find out what occupational therapists in these practice settings are using to help inform their practice, what the predominant focus is for assessment/intervention, what outcome measures are used and what the research priorities might be from an OT perspective? I had to make the change, as the work and fees put into being a limited company rather than just self employed are far too high considering I don't earn very much. Also, I'm looking to put a website together...any suggestions, to start with I was going to use a free site such as WiX or Wordpress. I'm finding not many people know what Occupational Therapy is! - Assessing Inattention/unilateral neglectIn Conditions + Specialisms·6 May 2020Hi everyone! I have had some downtime during this time of COVID-19 to look into some ways of improving my practice and how things run on the ward I work on, which is an inpatient stroke rehabilitation unit in New Zealand. I came across the Catherine Bergego Scale which addresses the extent of unilateral neglect (or inattention) and it has helped me as a tool for some sort of outcome measure. I find that I'll just be telling my team that "Mr E. has improved, he has done this, this and this today!" otherwise, and I have found that this scale has let me have a bit of a record of how he has improved. I find this measure easy to use, and can be done with everyday functional tasks (ie. I don't need any other equipment other than what we already have as it's all standard, functional items). Has anybody else used this before? - Anxiety performance at the Olympics.. what about performance and well-being as OTs?In Career Development·28 July 2021Today I read about Olympic athletes opening up about the relentless pressures of performing to their max and the mental health problems such as performance anxiety and depression as a result. I don't mean to compare OTs with Olympic athletes, but I know that some OTs work in very stressful environments, are expected to deal with a huge workload, perform too long beyond healthy limits, meanwhile trying to satisfy their clients and most likely their manager too. The question is: are you satisfied yourself? How long are you able to perform at this level? How long do you want to perform at this level? What do you need right now to stay happy and healthy? How do you prefer to work? How can you align your work with your values and life view? How can you make this happen? Sometimes we need to step back, take a breath and be honest with ourselves. When was the last time you checked in with your dreams in life? It can be a challenging process to re-consider your career. I know. In between travelling, working abroad, moving from The Netherlands to Ireland, I have had two careers: as a primary teacher and an OT. Now I'm combining private OT practice with being an OT Life Coach for people with Neurodiversity and for Healthcare Workers/OTs, as well as being a mentor for OTs who wish to use coaching as an OT intervention. And I'm loving it! All I can say is: be brave and ask the questions. Be honest and if you like to get honest support from a 'neutral person'... consider being coached to find your way! 'You're worth it!'15167

- OT + Brain Injury RehabilitationIn Conditions + Specialisms30 August 2021Such a cool and visual pictorial! Summarizes up what we assess and the complexity of cognition! I'm currently struggling a bit with being able to measure "initiation" - can only do it functionally and based on observation at the current time and exploring with my patient into what motivates and will cue her to get "up and going" especially when she goes home! Otherwise today I've used the COGNISTAT and the Rivermead Behavioural Memory test. All used alongside functional tasks to see how my patients would be affected with their goals for rehab :)22

I've been in post just over one year. Rates of dna are ++high so it's been difficult deciding between measures / assessments. I've just bought the role checklist and have always used the osa. With one guy who likes and is good at talking, I'm using the ocairs. Have spent most of last year looking into impact of head injury. This year though, have started looking into sensory processing (especially sensory seeking due to substance misuse). Happy to talk further on the topic. I'll watch here for your response (this is my first time using OT Hub so not au fait with how I will know you've responded). Good luck, it's a tricky client group to engage, takes about a year before trust is built I've found.22

I have signed up to the Health Education HCPC return to practice programme and am looking for a supervised practice placement. I wondered if this might be something any of you could possibly help me with? As I have been off the register for more than 5 years (I have not practiced as an OT since 2007) I will need to do around 30 hours of Supervised practice. This could mean me working with an OT as a volunteer for around 6 weeks or as an OT assistant/support worker. I know this is a difficult time but if you know of anyone who needs a spare pair of hands right now I would be keen to help (I hope to be more of a help than a hindrence)! I would be very grateful if you are able to help me with this or if you know anyone who might be able to. It really has, and continues to be so difficult for everyone, but particularly in the health and social care sector. I’ve been supporting healthcare professionals with my 1:1 programmes since 2017, and I’ve seen my clients find fulfilling and rewarding work that fits in well with home life too, but I’m very aware that for many, the cost of professional coaching is prohibitive. I’m looking to explore how I might deliver a programme that’s effective and accessible, but I would love to know what support would be most valued and make a genuine difference. I would really value the input of OTs from anywhere in the world, and I wondered if you might have 20 minutes over the coming week when we could schedule a call so I can understand issues of career and personal development from your perspective? - Getting into researchIn Research + Evidence·21 June 2018How many of you have read a journal article that implemented research and when you go to apply it in a clinic, school, home, hospital, or wherever you practice, realize that the assessment tool isn't available yet or you don't have enough details to really do what the research supported? I have been wondering about how to become more involved in research, but am not directly associated with Universities or Foundations that are asking for OT's to join their research teams! How do you suggest learning of projects that would welcome someone to their research team and help that individual learn to collect data or trial some treatment strategies or new assessment tool requiring field testing with real patients? Are there websites that actual list such needs, almost like a "classified advertisement for research wannabes?" Managing the work to retirement transition is currently an underdeveloped and under-researched area of practice within occupational therapy. If you are contemplating retirement? Or have you recently retired we need your help! Are you able to help? We have been developing a program for the past year to support the needs of people who are either about to retire or have recently retired from the perspective of the transition from work to retirement (W2R). The next step is to understand the needs of pre-retired and retired people and use the findings to further develop the W2R manual. The aim is to gather information about your experience of the retirement transition in your life. Whilst you are unlikely to personally benefit from this study your experience will enable us to develop a better intervention that is more likely to be of value to people preparing for retirement. This will increase the likelihood of its future use and the viability of it being an implementable intervention. Want to take part? To develop the manual, we are recruiting participants to take part in interviews which will be conducted online, we can use Zoom or MSMs Teams whichever you prefer, therefore travel is not required. - Multisensory Approaches For Special Needs Education Children'sIn Assessment + Evaluation·23 March 2022Finger tracing is a great strategy for early writers, for teaching pre-writing lines and shapes in preschool and toddler years, and for encouraging carryover of diagonal lines, jumping lines, and curves needed for form letters. Try a few of these finger tracing techniques to incorporate the tactile system into letter formation 1:Tracerace letters in a sensory bin using dry rice, sand, or shaving cream 2:Finger trace letters on sandpaper 3:Finger trace letters using a sensory bag (details to make this tool below) 3:Start with air writing letters 4:Draw a letter on the palm of the child’s hand. The student then needs to finger draw the same letter on the palm of their hand. 5:Finger trace letters on one another’s back 6:Draw letters with glue on paper. - ‘Almost’ Fully FledgedIn Student Spaces9 January 2020Its great to hear from a new OT, I love your passion and motivation. I would encourage you to embrace opportunities and have confidence in yourself. My first job was in acute mental health, I was surprised to get the job after my placement in mental health was in an untraditional setting. I spent some time here before moving to the LD team, again I had no placement experience but did my research and reinforced my passion for learning. Finally, mostly strangely I applied for a community nurse position, I knew I could fulfil the role with my mental health practitioner experience and focus on the psychosocial model. Yes I got the job, my point being never underestimate your transferable skills. For interviews I would say always research the service and their values, identify your key skills, strengths and weaknesses.
