Hi, I am an OT working in a Community Hospital. I am currently working alongside the nursing staff in creating a new night time intervention chart that gives a clear picture of Patients night time needs, gathers all relevant info, but is also quick and easy to fill out! Not wanting to re-event the wheel I imagine there are plenty already out there and was hoping that people may be able to send me some examples of what they use. Thank you so much in advance.

It’s great to be working with the nurses as they have a different model of practice / different model of recording info important to understand both language as it were... using something like maybe an enhanced care doc /behaviour chart or the existing day to day & night care plan but with added inform or filling it out enhanced better could work ? As kings as you have nursing & hca buy in and champions this will work.

@Kim Plumridge what a fab piece of work. This was something I obtained verbally on a morning nursing round as part of handover which they got used to sharing. I also have vast experience of reading “behaviour charts” which come in paper or electronic forms which were used for various reasons in rehab, intermediate care or acute hospital .. these were not well completed and needed a huge strategy on training on how to fill them out and what was relevant.
I would start with working out what you want to know from a chart when completed well by nursing or HCA’s and work backwards on how to achieve that .. do not underestimate the training in order to explain what and why required .. such a piece of work would be great for OTs working on discharges to advise on appropriate care, but also to social services to plan for and deliver appropriate care and monitoring as necessary.
