Assessment + Evaluation

Hiya I write many annual reviews for special school kiddies, however I have been asked to write 1 for a child I see in a mainstream school.

Does anyone have a template for these that they are happy to share?

Also as an independent OT I'm very realistic and don't push for over and above OT provision. So in your experience are parents quite accepting of changes in the EHCP from the Annual Review to reflect what the childs needs?
