Outcome measures within rapid response/urgent care in the community?
I am currently doing research on the importance of therapy within a MDT rapid response/community urgent care team, I am keen to get feedback on any outcome measures used within other rapid response services with therapy input?

Like Jamie. I used to work in Rapid response service and Rehab. I was based in London.
The outcome measure we used including Clinical Frailty Scale, Fall assessment, and the Barthel index. For us we would see patients multiple times within a week- so Barthel and CFS really helped.
